住院医师规范化培训模式效果的分析研究

【摘要】:研究目的 通过对培训效果的研究,发现住院医师规范化培训中出现的问题,并针对出现的问题,提出可行性建议,为完善住院医师规范化培训效果评价模式提供有益探讨,为医院管理者决策提供参考。 研究方法 通过引入柯氏四级评价方法即反应层、学习层、行为层、成果层对效果进行评价。通过文献法和专家访谈法制定调查问卷,主要进行满意度分析;学习层的主要是通过收集培训期间住院医师考核成绩、归属感等基本信息进行效果分析,结果进行t检验;行为层主要通过住院医师自评,科主任与护士长的他评来评价经过培训住院医师的行为是否发生改变,培训前后的分析使用配对t检验;成果层通过资料查阅搜集信息。 结果 反应层:住院医师对培训的总体满意度为4.05,在培训的师资、考核及薪酬满意度较低,而且博士较硕士而言满意度较低,科研型与临床型住院医师满意度差异无统计学意义。学习层:比较培训前后考试成绩差异有统计学意义。行为层:各项能力素质培训前后比较P值均小于0.01差异有统计学意义。成果层:此次参加培训的89名住院医师患者投诉率为0,事故率为0,执业医师考试通过率为100%,培养社会化住院医师共3人,就业率为100%。 结论 培训前后住院医师满意度,及各项成绩、能力素质均有提高,对组织也有较大贡献,同时通过数据表明住院医师规范化培训中存在评估结果缺乏反馈,教师积极性不高,薪酬待遇低,缺乏竞争机制及卫生法律法规知识较缺乏等问题。
[Abstract]:Research purpose Through the study of the training effect, we find the problems in standardized training of resident doctors, and put forward some feasible suggestions for the problems. In order to improve the evaluation model of standardized training for resident doctors, it provides a reference for hospital managers to make decisions. Research method Through the introduction of four evaluation methods, namely, reaction layer, learning layer, behavior layer and achievement level, the effect is evaluated. The questionnaire is made by literature and expert interview, and the satisfaction analysis is mainly carried out. In the learning layer, the results were analyzed by collecting the basic information, such as the assessment results and the sense of belonging, during the training period, and the results were analyzed by t-test. Behavior level mainly through resident self-evaluation, department director and head nurse to evaluate whether the behavior of the trained resident changes, the analysis before and after training using paired t test; The results layer collects information by consulting data. Results Response level: the overall satisfaction of resident doctors to training was 4.05, the satisfaction of teachers, examination and salary was lower in training, and the satisfaction of doctor was lower than that of master. There was no significant difference in the satisfaction of residents between the scientific research type and the clinical type. Learning level: there was significant difference in test scores before and after training. The difference of P value before and after the training was statistically significant. Results: the complaint rate of 89 resident patients who participated in the training was 0. The accident rate was 0, the passing rate of practicing physician examination was 100, the training of socialized resident was 3, the employment rate was 100. Conclusion Before and after training, residents' satisfaction, scores and abilities were improved, and they also contributed to the organization. At the same time, the data showed that there was a lack of feedback on the evaluation results in the standardized training of resident doctors. Teachers' enthusiasm is not high, salary is low, lack of competition mechanism and lack of knowledge of health laws and regulations.
