信任与自主:从分离走向融合——临床生命伦理学的视角

【摘要】：信任与自主的关系在不同的历史时期呈现不同的样态。传统家长主义模式,强调医生的绝对权威与患者的信任,拒绝患者自主;患者权利意识的觉醒,社会对医疗界不信任的增加,催生了患者自主,它成为医患之间对话的绝对权威。然而,自主的独立性、人际交互的陌生化以及信任的脆弱性,致使自主在某种程度上破坏了医患之间的信任。在现代性语境下,医患信任是实现患者自主的前提;自主的道德性为医患信任提供了基础,这二者为自主与信任的融合提供了可能。
[Abstract]:The relationship between trust and autonomy presents different patterns in different historical periods. The traditional paternalism model emphasizes the absolute authority of doctors and the trust of patients and rejects patient autonomy. The awakening of patients' consciousness of rights and the increase of social distrust in the medical field have given birth to patient autonomy. It becomes the absolute authority of the doctor-patient dialogue. However, the independence of autonomy, the strangeness of interpersonal interaction and the fragility of trust result in autonomy destroying the trust between doctors and patients to some extent. In the context of modernity, doctor-patient trust is the premise of realizing patient autonomy, and the moral character of autonomy provides the basis for doctor-patient trust, which provides the possibility for the integration of autonomy and trust.
