【摘要】：随着开封市人口老龄化程度日趋严峻,伴随老龄化而来的健康问题对现行养老和健康体系带来重大的挑战,其中失能老人的医疗护理与生活照料等需求以成为亟待解决的重要民生问题。因此,为破解失能老人养老难题,必须利用机构养老形式创新,鼓励养老机构增强医护功能,改善失能老人的健康状况,整合现有医疗服务与养老服务资源,推动新的医养结合形式的发展。以开封市作为调研样本,通过问卷调查和访谈法,对城区失能老人医养结合服务需求与供给现状进行分析,从中深刻剖析目前城市失能老人医养结合机构养老所存在的问题及原因。本文从实践中提取出医养结合机构养老的创新之处,就是以失能老人为目标群体,以养老机构下开设正规医疗诊所、养老机构和医疗机构相互合作等方式为目标群体提供养老和医疗服务,使其增加人性化的专业医疗服务。本文主要从五部分进行研究分析:第一部分为总括,对研究的背景和意义进行论述,梳理当前国内关于失能老人机构养老研究现状,以及国外医养结合养老的相关研究成果,阐述文章思路、方法和创新之处。第二部分回答了“什么是医养结合机构养老”的问题,以能力贫困、需要层次与福利多元主义为理论基础,从失能老人需求、养老服务提供主体等方面详细阐述了医养结合机构养老发展的依据。第三部分包括第三章和第四章内容,回答了“医养结合机构养老发展现状与前景如何”的问题,首先从总量上分析开封市养老服务提供匮乏,再通过实地调研结论,并梳理了医养结合养老机构的现状,整理出医养结合机构养老发展成就以及面临的主要困境,引出政策支持的必要性。第四部分回答了“如何破解失能老人医养结合机构养老的困境”的问题,以政策大力扶持作为解决失能老人医养结合机构养老困境的突破口,以培养“医养结合”护理人员为基础,依靠居家、社区养老的方式来减缓机构养老的压力,促使我国城镇化进程中失能老人养老问题解决。第五部分对全文主要结论进行了归纳,进一步提出医养结合机构养老发展进程中面临的问题,需要政府与社会力量共同参与解决。因开封市失能老人比重在河南省较高,逐渐推行医养结合机构养老新形式,在今后所有养老模式中发展进程中日益突出,也将成为政府和社会处理这类老年问题的新途径。为更好发展医养结合机构养老,需落实政府职责,多部门共同合作促进发展;鼓励社会力量参与并提供服务;提升养老机构医养结合自身实力,注重医疗服务投入等,并在认真结合开封市实情下,摸索出一条适合医养结合机构养老发展的道路。
[Abstract]:With the aging of the population in Kaifeng becoming more and more serious, the health problems associated with the aging of the population bring major challenges to the current pension and health system. Therefore, in order to solve the problem, it is necessary to make use of the innovation in the form of institutional pension to encourage the institution to enhance the health care function of the disabled elderly, so as to solve the problem of providing for the disabled, the needs of medical care and living care for the disabled are urgent to be solved. To improve the health status of the disabled elderly, to integrate the existing medical service and old-age service resources, and to promote the development of a new form of combination of medical care and maintenance. Taking Kaifeng City as a research sample, through questionnaires and interviews, This paper analyzes the demand and supply of medical and nursing services for the disabled elderly in urban areas. In this paper, the author analyzes the problems and causes of the aged in the medical and nursing institutions of the urban disabled elderly. This paper extracts from the practice that the innovation of the aged in the medical and nursing institutions is to take the disabled elderly as the target group. Providing old-age care and medical services to the target groups through the establishment of regular medical clinics under pension institutions, cooperation between pension institutions and medical institutions, etc. The first part is an overview of the background and significance of the study, combing the current domestic research on the disabled elderly institutions for the aged. The second part answers the question of "what is the old-age care system in combination of medical and support", which is poor in ability, Based on the theory of need level and welfare pluralism, this paper expounds in detail the basis for the development of the aged in combination of medical and nursing institutions from the aspects of the needs of the disabled elderly and the main body of providing services for the aged. The third part includes the third chapter and the content of chapter 4th. In this paper, the author answers the question of "what is the present situation and prospect of the development of the aged in the combination of medical and foster institutions". Firstly, it analyzes the shortage of the old-age service in Kaifeng from the total amount, and then through the field investigation conclusion, and combs the present situation of the old-age care institution of the combination of medical and support. This paper collates the achievements and the main difficulties of the development of the aged combined with medical institution and raises the necessity of policy support. In 4th, it partly answers the question of "how to solve the dilemma of the institution for the aged with disability". Taking the policy support as the breakthrough to solve the problem of the aged in the institution of medical care and raising for the disabled, taking the training of the nursing staff of the combination of medical treatment and support as the foundation, relying on the way of home and community old-age support, to alleviate the pressure of the institutional old-age care. In the 5th part, the main conclusions of this paper are summarized, and the problems faced in the process of the development of medical and nursing institutions for the aged are put forward. Because the proportion of disabled elderly in Kaifeng City is higher in Henan Province, the new form of old-age care in combination of medical and nursing institutions has been gradually implemented, which will become increasingly prominent in the development process of all pension models in the future. It will also be a new way for the Government and society to deal with such problems of old age. In order to better develop health care institutions for the aged, there is a need to fulfil the responsibilities of the Government and to work together in various sectors to promote development; and to encourage the participation of social forces and the provision of services; In order to improve the strength of medical and nursing institutions and pay attention to the input of medical services, and combining with the reality of Kaifeng City, this paper tries to find out a suitable way for the development of old-age care in combination of medical and nursing institutions.
