泰国清迈府缅甸籍劳工医疗服务研究

【摘要】：20世纪90年代以来,中南半岛的政治冲突和战争迫使大批来自越南、老挝、柬埔寨以及缅甸的难民涌入相对稳定的泰国。随着泰国经济的发展,劳动力的需求不断扩大,泰国劳动力无法满足经济发展的需求,泰国企业家们呼吁政府批准雇佣外籍劳动力,缅甸籍劳工成为这其中的主力军之一。清迈府是泰国北部比较大的城市,与缅甸掸邦接壤,许多缅甸籍劳工长期在清迈府工作和生活。据清迈府市政厅统计,2011至2015年5年间在清迈府注册的缅甸籍劳工累计达到36多万人。随着这些缅甸籍劳工在清迈长期工作生活,医疗服务逐渐成为最基本的生活需求之一,泰国各界也越来越关注缅甸籍劳工的医疗服务问题。本文以清迈府15个县的缅甸籍劳工(本文所指缅甸籍劳工是指取得泰国合法工作证的缅甸籍劳务人员)为主要样本,通过问卷调查对泰国清迈府缅甸籍劳工的医疗服务现状进行调查分析,包括政府的外籍劳工医疗政策、外籍劳工医疗权利、就医资源、就医服务以及缅甸籍劳工医疗意识等五个方面;从制度层面、社会层面、个人层面对缅甸籍劳工的医疗服务现状问题与原因进行分析。研究发现,缅甸籍劳工医疗服务存在的问题主要是医疗服务政策方面还有诸多局限、医疗权利不平等、就医资源不足、就医流程复杂、医疗费高等。对此,本研究从制度层面、社会层面、个人层面对改善清迈府缅甸籍劳工医疗服务提出了相关建议。首先,泰缅政府之间应加强合作,更加规范劳工的输出和输入;其次,清迈政府要更加重视外籍劳工群体,制定更符合清迈社会各群体的医疗政策,保障外籍劳工的医疗权利;最后,加强财政支持增加医疗资源,同时医疗机构也要培训好工作人员为外籍劳工提供更专业更优质的医疗服务。
[Abstract]:Since the 1990s, political conflicts and wars on the Indochina Peninsula have forced a flood of refugees from Vietnam, Laos, Cambodia and Myanmar into relatively stable Thailand. With the development of the Thai economy and the increasing demand for labor force, the Thai labor force cannot meet the needs of the economic development. Thai entrepreneurs have called for the government to approve the employment of foreign labor, and Burmese workers have become one of the main forces. Ching Mai is a relatively large city in northern Thailand, bordering Shan State, Myanmar, where many Burmese laborers have long lived and worked. More than 360,000 Burmese workers were registered in Qingmai between 2011 and 2015, according to the Qingmai City Hall. With these Burmese workers working and living in Chiang Mai for a long time, medical services have gradually become one of the most basic living needs, and more and more people in Thailand are paying attention to the issue of medical services for Burmese workers. The main samples of this paper are Burmese workers in 15 counties of Qingmai Prefecture (the Burmese workers in this paper refer to the Myanmar workers who have obtained a Thai legal work permit). A questionnaire survey was conducted to investigate and analyze the current situation of medical services for Myanmar workers in Chiang Mai Prefecture, Thailand, including the government's medical policy for foreign workers, medical rights of foreign workers, and medical access resources. From the system level, the social level, the personal level, the present situation and the reason of the medical service of the Burmese labor are analyzed in five aspects: the medical service and the medical consciousness of the Burmese labors, which are analyzed from the system level, the social level and the personal level. It is found that there are many limitations in medical service policy, such as unequal medical rights, insufficient medical resources, complicated process and high medical expenses. In view of this, this study puts forward some suggestions on how to improve the health care of Myanmar workers from the system level, the social level and the personal level. First of all, the Thai and Myanmar governments should strengthen cooperation to regulate the export and import of workers; secondly, the Chiang Mai Government should pay more attention to the foreign labor groups, formulate medical policies that are more consistent with the various groups of Chiang Mai society, and protect the medical rights of foreign workers. Finally, strengthening financial support and increasing medical resources, medical institutions should also train staff to provide more professional and quality medical services to foreign workers.
