经食管超声监测对肝移植术中容量管理的指导意义

【摘要】:目的:经食管超声心动图(TEE)已广泛应用于心脏外科等大型复杂手术术中检查、液体治疗、重症治疗等医疗领域。在国内,尚未见其应用于肝移植围麻醉期的相关报道。本研究旨在通过经食管超声多普勒监测指导肝移植患者术中容量管理,观察其对于改善血流动力学的临床效应。 方法:选择我院肝移植病例60例,其中男45例,女15例,ASA Ⅱ~Ⅲ级,肝功能Child-Pugh分级A或B级,年龄32~64岁,体重54-93kg。随机将其分为两组,对照组30例,TEE监测组30例。对照组以维持平均动脉压(MAP)、心率(HR)、中心静脉压(CVP)波动范围在基础值的20%以内,尿量在1ml/kg/h以上为补液原则。TEE监测组按照Sinclair等的方法,根据食管超声多普勒监测仪监测的纠正左室射血时间(LVETc)指导液体输入,控制LVETc于0.35s-0.40s之间。当LVETc小于0.35s时加快补液,当LVETc大于0.40s时减慢或暂停补液。记录切皮前即刻(T0)、手术开始后60min(T1)、无肝期10min(T2)、门静脉开放后10min(T3)、门静脉开放后60min(T4)、术毕(T5)平均有创动脉压(MABP)平均肺动脉压(MPAP)、中心静脉压(CVP)、心输出量(C0)。观察并分别记录患者术中输血输液量、尿量、心血管不良事件,包括血液动力学负性波动、心肌缺血。记录术后ICU停留时间、术后不良事件发生情况包括肺水肿、心肌梗死、肾功能衰竭、认知功能障碍、死亡等。 结果:两组患者于无肝前期(T。、T1)血流动力学指标(MABP, MPAP, CVP, CO)基本稳定;于无肝期(T2)血流动力学指标均明显降低,提示两种监测手段均无法避免门静脉及下腔静脉阻断所引起的血流动力学波动。对照组肝移植患者的血流动力学指标在新肝期各个时间点(T3,T4,T5)均有明显波动,相比于对照组,TEE监测组新肝期各个时间点血流动力学指标相对稳定,提示TEE指导容量管理可以改善肝移植术中剧烈的血流动力学变化。TEE组与对照组晶体、胶体、红细胞、血浆总输注量无差异,出血量无差异,TEE组患者术毕尿量多于对照组,术中和术后心血管不良事件发生率均明显低于对照组,术后脱机拔管时间与ICU停留时间TEE监测组少于对照组,提示TEE监测可以合理指导输液输血时机,改善包括肾脏、心脏在内的重要脏器灌注。 结论:通过食道超声指导肝移植术中容量管理,可以实时监测并准确快速地判断手术中循环血量的变化,能够有效维持血流动力学稳定,及时调控液体及血制品输注,降低液体负荷过重的风险,保证充足的组织器官灌注,同时还可以很好的降低肝移植围麻醉期不良事件发生率,减少术后拔管时间与ICU停留时间。
