无痛胃镜检查对不同体重患者呼吸循环功能的影响及护理对策

【摘要】：目的:观察不同体重患者在无痛胃镜检查时呼吸、循环功能的变化,评价相应护理措施的处理效果。方法:140例患者,根据体质指数分成3组:标准体重组、超重组与肥胖组。记录患者麻醉药物注射前、注药后睫毛反射消失时、胃镜检查操作时以及检查结束时各时间点的心率、血压、呼吸频率和末梢氧饱和度,并比较3组患者以上指标在采取护理措施前后的变化情况及各组间有无差异。结果:麻醉检查期间患者血压明显下降;但各组间血压、心率下降等循环指标变化差异无显著性。3组均有一定比例的患者出现氧饱和度下降。肥胖组需要托下颌和鼻咽通气道支持通气的比率显著增高。结论:无痛胃镜检查会不同程度地影响患者的呼吸功能,对肥胖患者影响尤为明显。置入鼻咽通气道可以有效改善患者检查期间的通气功能。
[Abstract]:Objective: to observe the changes of respiratory and circulatory function in patients with different weight during painless gastroscopy, and to evaluate the effect of corresponding nursing measures. Methods: 140 patients were divided into 3 groups according to body mass index (BMI): standard body weight group, overweight group and obesity group. The heart rate, blood pressure, respiratory rate and oxygen saturation at the end of gastroscopy were recorded before and after injection of anesthetic, when the eyelash reflex disappeared after injection, and at the end of gastroscopy. The above indexes of the three groups were compared before and after taking nursing measures, and there were no differences among the three groups. Results: the blood pressure of the patients decreased obviously during anesthesia examination, but there was no significant difference in blood pressure, heart rate and other circulatory indexes among the three groups. There was a decrease in oxygen saturation in a certain proportion of the patients in the three groups. The ratio of airway support ventilation to mandible and nasopharynx was significantly higher in obese group. Conclusion: painless gastroscopy can affect respiratory function in different degrees, especially in obese patients. Nasopharynx airway placement can effectively improve the ventilation function during examination.
