浮针联合红外照射疗法改善老年脑卒中后肩手综合征肩部疼痛的效果

【摘要】:目的探讨浮针联合红外照射疗法对老年脑卒中(中风)后肩手综合征(SHS)肩部疼痛的效果。方法在针灸门诊选取老年中风后SHS患者98例,随机分为观察组48例,浮针组30例,红外组20例,观察组予浮针联合红外照射疗法(在距离激痛点4~5 cm处进针,于皮下扫散运针,持续2 min,继以红外照射30 min),浮针组单纯予浮针疗法,红外组单纯予红外照射疗法,3组均予相同的康复训练,隔天治疗1次,连续治疗2 w。治疗前后采用视觉模拟评分法、高岸肩关节功能量表疼痛评分和改良Barthel指数对3组疼痛情况进行评估。结果观察组SHS肩部疼痛及生活自理能力的改善优于浮针组和红组(P0.05,P0.01);浮针组SHS肩部疼痛及生活自理能力的改善明显(P0.05);红外组SHS肩部疼痛改善明显(P0.05),生活自理能力改善不明显(P0.05)。结论浮针联合红外照射疗法可显著改善老年中风后SHS肩部疼痛,增强生活自理能力。
[Abstract]:Objective to investigate the effect of floating acupuncture combined with infrared radiation therapy on shoulder pain in elderly patients with shoulder-hand syndrome (SHS) after stroke. Methods 98 elderly patients with SHS after stroke were randomly divided into observation group (n = 48), floating needle group (n = 30) and infrared group (n = 20). After 30 mins of infrared irradiation, the floating needle group was treated only with floating acupuncture therapy, while the infrared group was treated with the same rehabilitation training once every other day for 2 weeks. Visual analogue scale, high bank shoulder function scale and modified Barthel index were used to evaluate the pain status of the three groups before and after treatment. Results the improvement of shoulder pain and self-care ability of SHS in observation group was better than that in floating needle group and red group, the improvement of shoulder pain and self-care ability of SHS in floating needle group was obvious (P 0.05), and that of SHS shoulder pain in infrared group was significantly improved (P 0.05), but the improvement of self-care ability was not obvious (P 0.05). Conclusion floating acupuncture combined with infrared radiation therapy can significantly improve the shoulder pain and self-care ability of elderly patients with SHS after stroke.
