中药联合乳房穴位按摩对肝郁气滞型产后缺乳的临床疗效观察

【摘要】：目的：本研究通过观察肝郁气滞型产后缺乳患者,在给予疏肝解郁、通络下乳中药的前提下加以运用乳房穴位按摩(手指点穴+乳房积乳疏通术),对产后缺乳患者肝郁气滞症候和缺乳情况的改善,以评价中药联合乳房穴位按摩对改善产后缺乳和肝郁气滞中医症候的有效性,进而验证其应用于临床的有效性,为治疗本病提供既有效又安全的治疗方法。方法：按照随机抽样的方法,收集河南中医药大学第一附属医院2015.2～2015.8期间,肝郁气滞型产后缺乳的住院患者。将符合纳入标准的126例患者以随机排列表法分为单纯中药治疗组(对照组)63例和中药联合乳房穴位按摩治疗组(治疗组)63例。两组均在产后第三天给与疏肝解郁、通络下乳中药治疗,治疗组在给与中药的基础上给与乳房穴位按摩(手指点穴+乳房积乳疏通术)治疗,五天一个疗程,依据疗前疗后中医证候积分和缺乳程度改善情况,比较单纯中药对照组与中药联合乳房穴位按摩治疗组的干预效果差异。结果：①肝郁气滞症候疗效的比较：治疗组与对照组疗前疗后中医肝郁气滞症候积分比较有显著的统计学意义(P0.05),且中药联合乳房穴位按摩治疗组疗效优于单纯中药治疗组；②缺乳程度的比较：两组疗前疗后缺乳程度积分比较有显著的统计学意义(P0.05),且中药联合乳房穴位按摩治疗组对缺乳程度积分的改善效果优于单纯中药治疗组；③中药联合乳房穴位按摩治疗组与单纯中药治疗对照组治疗后对产后缺乳的总疗效比较经统计学分析具有较明显的差异(P0.05),治疗后治疗组痊愈率为15.9%,对照组的痊愈率为1.6%,结果显示治疗组对肝郁气滞型产后缺乳的临床疗效优于对照组。结论：肝郁气滞型产后缺乳患者在给与疏肝解郁、通络下乳中药治疗的前提下配合河南中医药大学第一附属医院妇产科乳房穴位按摩方法治疗,能明显改善肝郁气滞证候情况；对乳汁分泌量的增加,乳房情况的改善,及保证乳汁通畅有很好的作用,且疗效明显优于单纯中药口服治疗,安全系数高,无毒副作用,值得临床推广。
[Abstract]:Objective: This study through the observation of liver qi stagnation in patients with postpartum hypogalactia, given Shugan Jieyu and traditional Chinese medicine under the premise of Tongluo milk to use breast massage (finger + breast milk product, deoppilation) on postpartum hypogalactia patients with liver qi stagnation syndrome and improve the situation of lack of milk, in order to evaluate the combination of traditional Chinese medicine breast massage effectiveness on improving postpartum hypogalactia and liver qi stagnation syndrome of traditional Chinese medicine, and to validate its application in clinical treatment of this disease, to provide a safe and effective treatment method. Methods: according to the method of random sampling, the First Affiliated Hospital of Henan University of traditional Chinese medicine was collected during the period of 2015.2 ~ 2015.8, and the hospitalized patients with stagnation of liver qi and qi stagnation type of postpartum lactation were collected. 126 patients who met the inclusion criteria were randomly divided into simple Chinese medicine treatment group (control group), 63 cases and traditional Chinese medicine combined with massage therapy group (63 cases). In the two group were given third days postpartum, Shugan Jieyu Tongluo milk under traditional Chinese medicine treatment, the treatment group were given based on the Chinese medicine to breast massage (finger + breast milk product deoppilation) treatment, a course of five days, according to before and after treatment of TCM syndrome integral and queru degree of improvement compared with control group, traditional Chinese medicine and traditional Chinese medicine combined with breast massage treatment group intervention effect difference. Results: the liver qi stagnation symptoms curative effect: treatment group and control group before and after treatment of TCM syndrome of stagnation of liver Qi scores were statistically significant (P0.05), and the traditional Chinese medicine combined with breast massage treatment group curative effect is better than pure Chinese medicine treatment group; the lack of comparative degree of milk: the two groups before and after treatment hypogalactia the degree of integration were statistically significant (P0.05), and the traditional Chinese medicine combined with acupoint massage therapy group on breast queru extent improve the integral effect is better than pure Chinese medicine treatment group; comparison of the statistical analysis showed significant differences in the traditional Chinese medicine combined with breast massage treatment group and medicine treatment of Dan Chunzhong total efficacy of the control group after treatment for lack of milk postpartum (P0.05) after treatment, the cure rate was 15.9%, the cure rate of the control group was 1.6%, the results showed excellent clinical efficacy in treatment group of liver qi stagnation in control of postpartum hypogalactia Group. Conclusion: liver qi stagnation type of postpartum hypogalactia patients with depression, under the premise of solution Tongluo milk of traditional Chinese medicine in the treatment given with Shugan Henan College of traditional Chinese Medicine First Affiliated Hospital of Obstetrics and Gynecology breast massage therapy can significantly improve the liver qi stagnation syndrome; to increase milk secretion, breast conditions improve and ensure milk smooth have very good effect, and the effect is better than pure Chinese medicine oral treatment, high safety, no toxic side effect, is worthy of promotion.
