强刺激手法治疗臀中肌综合征的临床疗效观察

【摘要】：目的:通过强刺激手法治疗对臀中肌综合征的临床疗效评价,并与口服非留体抗炎药治疗对照,筛选疗效更佳的臀中肌综合征治疗方法,以期能寻找疗程短、副作用小,复发率低、误诊率少的方法。方法:从2016年3月至2017年3月期间,根据诊断标准、纳入标准从浙江中医药大学附属第三医院推拿科门诊中纳入60例患者,根据随机化(Block randomization)的分组原则,把病例分成2个组:治疗组30例,运用强刺激手法(治疗隔日1次治疗,治疗5次,其治疗周期为10天),对照组30例,口服非甾体抗炎药治疗(饭后服用塞来昔布胶囊为200m g,1日1次,其治疗周期为10天)。规范各种治疗方法、疗程,运用疼痛视觉量表(VAS)观察评分、JOA下腰痛疾患疗效评定标准、Oswestry功能障碍指数,进行疗前、疗后对照,确立临床疗效评价。结果:治疗组30例臀中肌综合征患者中,28例获得临床痊愈,2例有效,总有效为100%,获得临床痊愈的治疗次数最少者为1次,最多者为5次。对照组30例臀中肌综合征患者中,12例获得临床痊愈,15例有效,3例无效。治疗组30例臀中肌综合征患者治疗后的(VAS)观察评分、JOA下腰痛疾患疗效评定标准、Oswestry功能障碍指数,与治疗前比较均有显着性差异。治疗组与对照组中均为出现不能坚持治疗者或出现严重不良反。结论:通过解剖学原理,准确找到臀中肌的损伤部位,予以强刺激手法治疗,可以快速消除或减轻症状,起到解痉止痛的效果,使患者临床痊愈。
[Abstract]:Objective: to evaluate the clinical effect of strong stimulation and manipulation on gluteal middle muscle syndrome, and compare it with oral non-body anti-inflammatory drugs to screen the better treatment method for gluteal middle muscle syndrome, in order to find a shorter course of treatment and less side effect. The method of low recurrence rate and low misdiagnosis rate. Methods: from March 2016 to March 2017, 60 patients were included in the clinic of Tuina Department of the third affiliated Hospital of Zhejiang University of traditional Chinese Medicine according to the diagnostic criteria and the principle of randomized (Block randomization) grouping. The patients were divided into two groups: treatment group (30 cases) and control group (30 cases). Oral non-steroidal anti-inflammatory drugs (celecoxib 200 mg, once a day, 10 days after meals). Standard treatment methods, course of treatment, using pain visual scale (VAS) observation score, JOA low back pain disease evaluation standard, Oswestry dysfunction index, pre-treatment, post-treatment control, to establish clinical efficacy evaluation. Results: in the treatment group, 28 cases were clinically cured, 2 cases were effective, the total effective rate was 100, the least number of times of clinical cure was 1, and the most frequent was 5 times in the treatment group of 30 cases of gluteal middle muscle syndrome. In the control group, 12 cases were cured, 15 cases were effective and 3 cases were ineffective. In the treatment group, there were significant differences between 30 patients with gluteal middle muscle syndrome (GMS) after treatment and before treatment by observing (VAS) score, evaluating the efficacy of low back pain under JOA, and the index of Oswestry dysfunction. In both the treatment group and the control group, patients who could not adhere to the treatment or severe adverse reactions were found. Conclusion: through anatomical principle, finding the injured part of gluteus middle muscle accurately and giving strong stimulation and manipulation treatment can quickly eliminate or alleviate symptoms, play the effect of relieving spasmolysis and relieving pain, and make the patients recover clinically.
