公共管理视角下中国医疗卫生体系改革问题研究

【摘要】：医疗卫生体系的建立健全是保障人民安居乐业的重大的民生问题,是国民经济稳定快速运营的“稳定器”,是实现中华民族伟大复兴的中国梦的有利保障。目前,世界上每个国家都有自己的医疗卫生体系,而要形成一套兼顾公平与效率的,得到社会各方认可的高效运作的医疗卫生体系则是一个世界性的难题。各国面临的国情、医疗卫生体系的基础、背景均不相同,没有那个国家可以完整的复制另外一个国家的医疗卫生体系。新形势下我国医疗卫生体系改革面临很多体制、机制上的问题,本文拟通过与其他国家医疗卫生体制、机制的对比,结合我国目前实际情况,为我国建立健全医疗卫生体系的改革,提供一些个人见解、建议。从建国后,新中国在医疗卫生体系改革中,为提升全民医疗保障、提供公共卫生服务经过多轮改革,形成了目前以“城镇职工基本医疗保险”、“城镇居民基本医疗保险”、“新型农村合作医疗”为主体的覆盖全民医疗保障体系,公共卫生服务体系也伴随着经济增长得到了快速提升,整体卫生条件、医疗卫生服务能力有了大幅提升。但是在医疗保障体系搭建过程中效率与公平的方面、在医疗保障体系与卫生服务体系之间相互衔接,以及政府、卫生行政部门在整个医疗卫生体系运转过程中仍存在很多问题。“看病难”、“看病贵”、“医患关系紧张”等问题依然突出。本文结合我国目前的实际情况,分别从医疗保障体系、医疗卫生服务体系两大方面,对比、分析、借鉴各国的医疗卫生体系运行情况,从国家的重视程度、医疗保障体系付费方、医疗保障与医疗服务关系、医疗卫生政策形成机制、第三方非盈利组织缺失以及在医疗卫生服务体系中看病难、看病贵、医患关系紧张以及医疗服务定价等方面寻找问题,并针对问题进行国内外对比分析。并汇总问题及其产生问题的成因,从八个方面给出建议。
[Abstract]:The establishment and perfection of the medical and health system is an important livelihood issue for the people to live and work in peace and contentment, a "stabilizer" for the stable and rapid operation of the national economy, and a favorable guarantee for Chinese Dream, which realizes the great rejuvenation of the Chinese nation. At present, every country in the world has its own medical and health system, and it is a worldwide problem to form a set of efficient and efficient medical and health system which takes account of fairness and efficiency. Each country faces different national conditions, the basis and background of the health system, and no country can copy the health system of another country in its entirety. Under the new situation, China's medical and health system reform is faced with many problems in system and mechanism. This article intends to compare with other countries' medical and health system and mechanism, and combine with the actual situation of our country at present. To establish and improve the health care system reform, provide some personal views and suggestions. After the founding of the people's Republic of China, in the course of the reform of the medical and health system, in order to enhance the medical security of the whole people and provide public health services, after many rounds of reforms, the current "basic medical insurance for urban workers" and "basic medical insurance for urban residents" have been formed. The "New Rural Cooperative Medical system" covers the national medical security system, and the public health service system is also accompanied by rapid economic growth, the overall health conditions, medical and health service capacity has been greatly improved. But in the aspect of efficiency and fairness in the process of setting up the medical security system, there are still many problems in the operation of the whole medical and health system, such as the connection between the medical security system and the health service system, as well as the government and the health administration department. Problems such as "difficult to see a doctor", "expensive to see a doctor" and "tense relationship between doctors and patients" remain prominent. According to the actual situation of our country, this paper compares, analyzes and draws lessons from the operation of the medical and health systems of various countries from the two aspects of the medical security system and the medical and health service system, as well as the degree of national attention to the operation of the medical and health system. The payer of medical security system, the relationship between medical security and medical service, the mechanism of forming medical and health policy, the absence of third-party non-profit organizations, and the difficulty and high cost of seeing a doctor in the medical and health service system, The relationship between doctors and patients and the pricing of medical services are analyzed and compared at home and abroad. And summarizes the problem and the causes of the problem, from eight aspects to give suggestions.
