成都市大学生基本医疗保险制度实施状况的调查研究

【摘要】：国家的“新医改”把大学生也纳入城镇居民基本医疗保险中来，不仅可以扩大社会互助共济的范围，提高大学生的医疗保障水平，也可以减轻学生家庭因病造成的经济负担，利于体现社会公平。在政府的干预之下，成都市大学生基本医疗保险制度已建立并运行起来，但从大学生基本医疗保险制度推进以来的实践表明，现行的大学生基本医疗保险制度设计及相关配套措施没有切实解决对医疗服务供求双方的行为约束问题，致使道德风险严重，大学生基本医疗保险制度运行绩效低下。本文将在认清目前成都市大学生基本医疗保险制度的发展历程的基础上，通过对成都市大学生基本医疗保险制度实施状况的深入分析，找出大学生基本医疗保险制度运行中存在的现实问题，借助公共管理学的相关理论工具，找出当前制度无效率的根源，为完善成都市大学生基本医疗保险制度提供建议。本文得出的主要结论是：政府作为社会的管理者和公共事业的组织者，其核心的一个职能就在于保证公共服务供给的秩序，保护公共需求的实现机制。（1）从大学生的实际保障需求和意愿出发来设计完善大学生基本医疗保险制度，针对不同群体的医疗保障需求，构建大学生社会医疗保险混合体系；（2）加强过程监督，强化政府在大学生基本医疗保险制度中的责任；（3）明确校医院的职责定位，加速推进校医院的体制改革；（4）强化宣传，加强健康教育，，提高大学生风险防控意识。本文在研究内容上，将成都市具体的结构性环境与政策联系起来进行系统分析，以成都市大学生基本医疗保险制度实施情况的问卷调查结果作为数据支撑，深入分析成都市大学生基本医疗保险制度在时代背景下的具体意义和运行结果，提出完善成都市大学生基本医疗保险制度的建议；在研究方法上，将运用公共管理、公共服务、公共政策执行等相关理论方法，以问卷分层抽样调查法为主，辅助文献研究法和实地观察法，从公共政策本身、公共政策执行对象、公共政策执行环境等方面，分析影响成都市大学生基本医疗保险制度运行的阻碍因素；在研究结论上，基于成都市具体的环境和高校学生的特征分析，结合国际国内大学生保险制度的成功经验，提出了完善成都市大学生基本医疗保险制度的对策，具有较强的实践指导意义。
[Abstract]:The "new medical reform" of the state will also bring college students into the basic medical insurance for urban residents, which can not only expand the scope of social mutual aid, raise the level of medical security for college students, but also alleviate the economic burden caused by the illness of the students' families. Under the intervention of the government, the basic medical insurance system for college students in Chengdu has been established and operated, but the practice since the promotion of the basic medical insurance system for college students shows that, The current design of the basic medical insurance system for college students and related supporting measures have not effectively solved the problem of behavior constraints on both sides of the medical service supply and demand resulting in serious moral hazard and low performance of the basic medical insurance system for college students. Based on a clear understanding of the development of the basic medical insurance system for college students in Chengdu, this paper analyzes the implementation of the basic medical insurance system for college students in Chengdu. This paper finds out the practical problems existing in the operation of the basic medical insurance system for college students, with the help of the relevant theoretical tools of public management, finds out the root causes of the inefficiency of the current system, and provides suggestions for perfecting the basic medical insurance system for college students in Chengdu. The main conclusion of this paper is that the government, as the administrator of the society and the organizer of the public utilities, has the core function of ensuring the order of the public service supply. The realization mechanism of protecting public demand. 1) to design and perfect the basic medical insurance system of college students according to the actual demand and willingness of college students, aiming at the medical security needs of different groups. In this paper, the specific structural environment and policy of Chengdu are systematically analyzed, and the results of questionnaire survey on the implementation of the basic medical insurance system for college students in Chengdu are taken as the data support. This paper deeply analyzes the specific significance and operation results of the basic medical insurance system for college students in Chengdu under the background of the times, and puts forward some suggestions for perfecting the basic medical insurance system for college students in Chengdu. Related theoretical methods such as public policy implementation, mainly questionnaire stratified sampling survey, auxiliary literature research and field observation, from the public policy itself, public policy implementation object, public policy implementation environment, etc. Based on the analysis of the specific environment of Chengdu and the characteristics of college students, combined with the successful experience of international and domestic college students' insurance system, this paper analyzes the factors that hinder the operation of the basic medical insurance system for college students in Chengdu. This paper puts forward the countermeasures to perfect the basic medical insurance system for college students in Chengdu, which is of great practical significance.
