徐州市推进医疗保险“三保合一”的困境与出路研究

【摘要】：基本医疗保险“三保合一”是我国社会保障体系改革和发展的重要环节,是人民权利平等和共享改革发展成果的重要体现。党的十八大报告和十八届三中全会中都提出要统筹推进城乡社会保障体系建设,整合城乡居民基本医疗保险,这是我国经济社会发展的现实需求。目前三大基本医疗保险制度发展水平存在较大差距,解决分立的基本医疗保险制度存在的冲突和问题是研究医疗保险整合的重要原因。现在的经济社会背景下,对于徐州市基本医疗保险“三保合一”的研究,对徐州市医疗保险整合的理论和现实发展具有重要意义,对于我国的医疗保险改革也有一定的借鉴意义。文章运用文献研究、系统研究和逻辑历史统一、调查研究等科学研究方法,收集相关资料进行整理分析。通过对于徐州市城镇职工基本医疗保险、城镇居民基本医疗保险和新型农村合作医疗三大医疗保险模块的发展现状分别进行调查研究,指出当前徐州市医疗保险制度取得的成果以及发展过程中由于医疗保险制度“碎片化”带来的问题,得出医疗保险“三保合一”的迫切形势。结合我国其他试点地区医疗保险整合发展的实际状况,归纳总结出适合徐州市当前发展需要的医疗保险“三保合一”的有利经验。文章重点分析了徐州市医疗保险“三保合一”目前面临的思想意识、法律制度层面以及实际操作中的一些具体困难,并指出这些问题背后潜在的原因,试图针对这些具体问题提出相应的解决对策。文章最后得出结论:徐州市的基本医疗保险“三保合一”是一个长期的发展过程,不能盲目求快,应该实施“分步走”的战略。通过对于医疗保险整合理论研究的不断完善,逐步解决医疗保险整合的缴费待遇标准、转移续接和管理部门统一等问题,根据当时当地具体的经济发展水平循序渐进、统筹兼顾,逐步推进徐州市基本医疗保险整合。
[Abstract]:The basic medical insurance is an important link in the reform and development of the social security system in China. It is also an important embodiment of the equal rights of the people and the sharing of the fruits of reform and development. The report of the 18th National Congress of the CPC and the third Plenary session of the 18th CPC Central Committee both proposed that the construction of urban and rural social security system should be promoted as a whole and the basic medical insurance for urban and rural residents should be integrated, which is the realistic demand for the economic and social development of our country. At present, there is a big gap in the development level of the three basic medical insurance systems. It is an important reason to study the integration of medical insurance to solve the conflicts and problems existing in the separate basic medical insurance system. Under the current economic and social background, the research on the integration of the three guarantees of basic medical insurance in Xuzhou is of great significance to the theoretical and practical development of the integration of medical insurance in Xuzhou. Also has certain reference significance to our country's medical insurance reform. In this paper, scientific research methods such as literature research, systematic research and logic history unification, investigation and research are used to collect relevant data for analysis. Through the investigation and research on the development status of the basic medical insurance for urban workers, the basic medical insurance for urban residents and the new rural cooperative medical insurance in Xuzhou, The paper points out the achievements of the medical insurance system in Xuzhou and the problems caused by the fragmentation of the medical insurance system in the course of its development. Combined with the actual situation of the integration and development of medical insurance in other pilot areas in China, this paper summarizes the beneficial experience of "three guarantees in one", which is suitable for the current development needs of Xuzhou City. This paper focuses on the analysis of the ideological consciousness, the legal system and the practical difficulties faced by Xuzhou medical insurance, and points out the potential reasons behind these problems. This paper tries to put forward corresponding solutions to these specific problems. Finally, the paper draws a conclusion that the basic medical insurance in Xuzhou is a long-term development process, which should be carried out step by step instead of blindly seeking speed. Through the continuous improvement of the theoretical research on the integration of medical insurance, gradually resolving the standard of payment treatment for medical insurance integration, transfer continuity and unification of management departments, etc., according to the specific level of local economic development at that time, step by step. Overall consideration, gradually promote the integration of basic medical insurance in Xuzhou.
