银色经济条件下的医疗服务体系重构——辨析老年长期照护与医疗服务的关系

【摘要】：健康长寿的社会常态刺激了老年医养服务需求,从轻到重的排列包括日常照料、失能护理、慢病管理、病后康复、急症诊疗,有病与非病、医与非医、残与非残的区别。一概而论地谈老年长期照护,可能导致资源配置和服务供给与需求的错位。本文坚持"分类以提高质量"和"整合以降低成本"的原则,将老年长期照护需求分为老化失能照料、病残失能护理、失智老年人长期照护三类,并从持续天数、实施场所、服务内容、支付方式、受益人群和支付工具六个维度对比老年长期照护与老年急症诊疗和后期康复、慢病管理的差异,以分辨各类老年长期照护项目的内涵和外延,在建立老年长期照护制度的同时引导国家医疗服务体系的重构。
[Abstract]:The normal state of health and longevity stimulates the demand for medical and nursing services for the elderly, including daily care, disability nursing, management of chronic illness, rehabilitation after illness, emergency diagnosis and treatment, sick and non-sick, medical and non-medical, and the difference between disabled and non-disabled. A general discussion of long-term care for the elderly may lead to misalignment of resource allocation and service supply and demand. Based on the principles of "classification to improve quality" and "integration to reduce cost", the long-term care needs of the elderly are divided into three categories: aging disability care, and long-term care for the mentally retarded elderly. In order to distinguish the connotation and extension of various items of long-term care for the elderly, six dimensions of service content, payment method, benefit group and payment tool were compared between the elderly long-term care and the elderly emergency diagnosis and treatment, the later rehabilitation, the chronic illness management, and the difference between the six dimensions of service content, payment method, benefit group and payment tool. The establishment of long-term care system for the elderly at the same time guide the reconstruction of the national medical service system.
