医患关系视角下医生职业倦怠及对策

【摘要】:随着医疗改革的深入发展,医患关系越来越受到人们的关注,每年不断增加的医疗暴力事件反映着医生与患者之间的关系越来越紧张。医患关系的不和谐是伴随着医疗改革中医院不完全市场化而产生的,医患矛盾的增加无形中给医生在执业过程中增加了压力,加上医疗资源分布不均衡,医院体制不完善,增加了医生行业的职业倦怠。当医生产生了职业倦怠以后,会影响到工作,更会影响到医患关系的发展。这样医患矛盾与医生的职业倦怠会陷入恶性循环,使得两者关系越来越糟糕。医患矛盾不仅能影响医生的职业倦怠,更能间接的反映出医生的职业倦怠情况。医患矛盾的产生大部分是由于医生的大处方、滥检查,医患之间沟通不畅,医疗效果不满意,患者认为医生刻意隐瞒病情,医疗事故无处投诉,医院与医生相互偏袒等等,而其中部分原因也是医生产生职业倦怠的影响因素。 针对上诉问题,本文采用了理论与实践相结合的方法,通过翻阅大量文献资料以及询问包括医生在内的相关人士的一些问题,从多个层次、多个角度、多个方位分析医生倦怠的原因,力求从客观公正的角度来分析问题,从宏观上分析了医生的就业形势严峻,就业趋势是偏向大城市的三级医院就业,国家相应的法律法规在医疗以及医生方面的不完善,国家的政策没有鼓励性偏向,从中观上分析了当前医生的工作环境,医院管理制度上的缺陷,激励制度的不合理,还有从微观上分析了医生所处行业的特殊性,其劳动价值以及劳动所得不相符合等问题。并试图利用新公共管理这方面的理论有针对性的提出相关对策及建议,分别从医院方面以及政府方面两个方向提出各项改革措施,目的在于引导医院改革方向以及政府政策的偏向,来从多方面、有效的减少医生的职业倦怠问题,从而改善医患矛盾,建立和谐的医患关系。所以解决医生的职业倦怠不仅能有效的推进医改,还能为建立和谐社会添砖加瓦,为最终实现中国梦更迈进一步。
[Abstract]:With the development of medical reform, people pay more and more attention to doctor-patient relationship. The increasing incidents of medical violence every year reflect the growing tension between doctors and patients. The disharmony between doctors and patients is accompanied by the incomplete marketization of traditional Chinese medicine hospitals. The increase of doctor-patient contradictions has invisibly increased the pressure on doctors in the process of practicing, coupled with the imbalance in the distribution of medical resources and the imperfection of the hospital system, which has increased the job burnout of the doctor profession. When the doctor produces job burnout, It will affect the work and the development of the doctor-patient relationship. In this way, the doctor-patient conflict and the doctor's job burnout will fall into a vicious circle, which will make the relationship worse and worse. The doctor-patient conflict can not only affect the doctor's job burnout, but also affect the doctor's job burnout. The conflicts between doctors and patients are mostly due to doctors' large prescriptions, excessive examination, poor communication between doctors and patients, unsatisfactory medical effects, and patients' belief that doctors deliberately conceal their condition. Medical malpractice has no place to complain, hospitals and doctors favor each other, and some of the reasons are also the influence factors of doctors' job burnout. In view of the appeal problem, this paper adopts the method of combining theory with practice, through reading a large amount of literature and asking some questions of relevant people, including doctors, from many levels and from many angles. By analyzing the causes of doctors' burnout in various directions, and trying to analyze the problems from an objective and impartial perspective, the paper analyzes the grim employment situation of doctors from a macro perspective. The employment trend is that the employment trend is in favour of tertiary hospitals in big cities. The state's corresponding laws and regulations in the medical treatment and doctors are not perfect, the national policy has no encouraging bias, from a meso analysis of the current doctor's working environment, hospital management system defects, incentive system unreasonable, It also analyzes the particularity of doctors' profession, its labor value and labor income, and tries to make use of the theory of new public management to put forward relevant countermeasures and suggestions. In order to guide the direction of hospital reform and the bias of government policy, the reform measures are put forward from the hospital and the government, respectively, in order to effectively reduce the problem of doctors' job burnout from many aspects. In order to improve the contradiction between doctors and patients and establish a harmonious relationship between doctors and patients, resolving doctors' job burnout can not only effectively promote the medical reform, but also contribute to the establishment of a harmonious society, and make further progress for the ultimate realization of Chinese Dream.
