中卫市农村卫生服务体系建设研究

【摘要】：农村卫生服务体系由政府、集体、社会和个人举办的县、乡、村三级医疗卫生机构组成,以县级医疗卫生机构为龙头,乡(镇)卫生院为中心,村卫生室为基础。主要包括县医院、县中医(民族医)医院、县疾病预防控制机构、县卫生执法监督机构、县妇幼保健机构、乡(镇)卫生院、村卫生室及其他卫生机构等,是我国农村居民获得基本医疗服务和公共卫生服务的重要支柱。国内研究学者主要是针对农村卫生服务的政府投入方面、人力资源管理方面、绩效评价体系方面、组织建设方面进行研究,对农村卫生服务体系建设方面的详细研究相对较少。因此研究农村卫生服务体系建设对于目前农村医疗卫生服务体系建设有着重要意义。本文主要通过对医疗卫生服务体系相关理论的研究,阐述了农村卫生服务体系的框架构成,在运用新公共管理理论和社会保障理论的基础上,结合中卫市农村卫生服务体系建设的实际情况,采用文献法及访谈法对中卫市农村卫生服务体系建设进行深入研究。首先,文章介绍了中卫市农村卫生服务体系建设的基本现状,指出当前中卫市农村卫生服务体系建设方面取得的成效,比如在基础设施建设、公立医院改革、卫生信息化等方面有了长足的发展。其次,从机构设置、人才队伍建设、服务能力、专科建设等方面介绍了中卫市农村卫生服务体系的基本情况;同时指出了中卫市农村卫生服务体系目前存在的问题,诸如对农村卫生服务体系重要性认识不足、基层服务能力有待加强、医院债务负担较重、医疗市场竞争日益加剧等问题,针对这些问题产生的原因进行了深入分析,存在着对农村卫生服务体系建设的重要性认识不足、责权不明晰、投入不足、机制不健全以及人才管理政策等几个方面。在对国内外先进地区农村卫生服务体系建设的经验分析的基础上从以下几个方面提出了相应的措施:编制好“十三五”发展规划、深化医药卫生体制改革、大力推进卫生信息化建设、加大人才队伍建设、加强基层医疗服务体系建设、全面提升诊疗服务水平等。
[Abstract]:The rural health service system is composed of county, township and village medical and health institutions organized by the government, collective, society and individuals, with county-level medical and health institutions as the lead, township (town) health centers as the center and village clinics as the basis. Mainly including county hospitals, county traditional Chinese medicine (ethnic medicine) hospitals, county disease prevention and control agencies, county health enforcement supervision agencies, county maternal and child health care institutions, township (town) hospitals, village clinics and other health institutions, etc. It is an important pillar of rural residents' access to basic medical services and public health services. Domestic scholars mainly focus on the government input of rural health services, human resources management, performance evaluation system, organizational construction research, rural health service system construction of detailed research is relatively small. Therefore, it is of great significance to study the construction of rural health service system. This article mainly through the research to the medical and health service system related theory, elaborated the rural health service system frame constitution, in the use of the new public management theory and the social security theory foundation, Combined with the actual situation of the construction of rural health service system in Zhongwei city, the paper makes a deep study on the construction of rural health service system in Zhongwei city by using the method of literature and interview. First of all, the article introduces the basic status quo of the construction of rural health service system in Zhongwei City, and points out the achievements in the construction of rural health service system in Zhongwei City, such as the construction of infrastructure, the reform of public hospitals, Health informatization and other aspects have made great progress. Secondly, this paper introduces the basic situation of the rural health service system in Zhongwei City from the aspects of institution setting, talent team building, service ability and specialty construction, and points out the existing problems of the rural health service system in Zhongwei City. Such problems as insufficient understanding of the importance of rural health service system, need to be strengthened in basic level service capacity, heavy hospital debt burden, increasing competition in the medical market, and so on, are analyzed in depth in view of the causes of these problems. There are some aspects such as insufficient understanding of the importance of the construction of rural health service system, unclear responsibility and power, insufficient investment, imperfect mechanism and talent management policy. Based on the analysis of the experience in the construction of rural health service system in advanced areas at home and abroad, the corresponding measures are put forward in the following aspects: making a good development plan for the 13th Five-Year Plan and deepening the reform of the medical and health system. Vigorously promote the construction of health information, strengthen the construction of talent team, strengthen the construction of grass-roots medical service system, and improve the level of diagnosis and treatment services in an all-round way.
