医患交际中陈述患者就诊理由的会话分析

【摘要】:据一位新西兰门诊医生讲,“为了能获得和医生有说话的权力,你必须满足一个前提条件,那就是你生病了。这样,你就可以去找他,让他履行作为医生的职责。” 随着会话分析研究方法的兴起和发展,越来越多的国内外学者致力于对日常会话的研究;不仅如此,国外的许多学者已将会话分析的基本理论应用到了医患谈话的探讨中来,然而,此方面研究的重点大都限于医生和患者双方,尤其是医生这一方面。很少有研究涉及到病人在就诊时陈述就诊理由对医患交际的作用。在我国,病人在陈述就诊理由是一个非常普遍的现象。所以研究病人陈述就诊理由有着重要的现实意义。 运用会话分析的研究方法,本文深入研究了病人陈述就诊理由在医患交际中所起的作用,关注病人是如何描述他们的医疗问题,如何来证明他们来就医是正当的。 通过对中国门诊室中自然发生的医患会话进行录音转写和分析,作者在文中阐述了病人陈述就诊理由的一般形式,这种交际的话轮设计和医生的回应方式。在接下来的文章中,作者论述了病人主要通过作诊断陈述,在决策的过程中引入第三方,和医生对病情进行解释三个方面来陈述自己的就诊理由。作者在文章中分别通过语料对这三方面病人陈述就诊理由的过程做了深入的分析。 当然,把来医院就医正当化并不是在问题展示阶段就结束,同样的陈述也会出现在病情陈述阶段和诊断的过程中。正是因为这个原因,很多涉及到病人合理的出现在医生办公室的问题会在这儿处理,而且,这也正是为什么医生合理的管理这一阶段有非常的意义。 最后作者提出了目前研究的不足以及对未来研究的建议。
[Abstract]:According to a New Zealand outpatient, "in order to have the right to speak to a doctor, you have to meet a condition that you are sick." Then you can go to him and let him perform his duties as a doctor. " With the rise and development of conversational analysis research methods, more and more scholars at home and abroad have devoted themselves to the study of daily conversation. Moreover, many foreign scholars have applied the basic theory of conversational analysis to the discussion of doctor-patient conversation. However, the focus of this study is limited to both doctors and patients, especially doctors. Few studies have addressed the role of patients' reasons in doctor-patient communication. In our country, it is very common for patients to state their reasons for seeing a doctor. Therefore, it is of great practical significance to study the patient's reasons for seeing a doctor. Using the method of conversational analysis, this paper makes an in-depth study of the role of patients' statements in doctor-patient communication, focusing on how patients describe their medical problems and how to justify them to seek medical treatment. By recording and analyzing the naturally occurring doctor-patient conversation in the Chinese out-patient clinic, the author expounds the general form of the patient's statement of the reasons for seeing a doctor, the design of the communication turn and the response of the doctor. In the following article, the author discusses that the patient mainly states his reasons by making diagnostic statements, introducing third parties into the decision-making process, and explaining the condition of the patient. In this paper, the author makes an in-depth analysis of the process of presenting the reasons of the three aspects through the corpus. Of course, the justification of coming to the hospital does not end at the problem presentation stage. The same statement also appears in the condition statement stage and the diagnosis process. It is for this reason that many of the problems that involve patients reasonably appearing in the doctor's office are dealt with here, and that is why it is very meaningful for doctors to manage this stage properly. Finally, the author puts forward the deficiency of current research and suggestions for future research.
